Status: Full Time
Shift: 7:30am to 4:00pm
Summary
The Stroke Coordinator is responsible for the overall management of
the stroke program ensuring on-going quality of care delivered to
the stroke patient from admission through patient disposition.
Executes PI initiatives in order to promote optimal stroke care.
Provides assistance to staff conducting stroke alert overseeing
quality measures. Monitors all patient care documentation to
include ED documentation (including ED nurses notes, ED order sets
or pathway documentation, ED physician notes, ED record, ED triage
sheet, Registration form, ED vital signs graphical record).Ensures
an active community education program exists. Responsible for
compliance with regulatory agency and national standards for stroke
care.
Responsibilities
Meetings: Plan and develop all Stroke Management Council, Peer, and
other meetings as needed. Manages the total care of a stroke
patients by ensuring evidence based measures are met through out
patients stay and discharge. Attends annual stroke seminars,
participates in American Heart Association (AHA), The Joint
Commission, Webinar, utilizes JEN for CEUs and maintains NIH stroke
scale certification. Protocols, Orders, Policy and Procedures:
Develops, maintains, revises, and updates Stokes specific
guidelines. Continually monitors and assesses teams adherence to
The Joint Commission (TJC) stroke specific standards. Staff
Education: Serves as clinical expert on Neurological patients to
staff, physicians and ancillary staff. Coordinates education,
evaluations needs, and develops plans for training for all staff
(also insures stroke team maintains minimal CE standards required
by The Joint Commission. Ensures education materials are up to date
and readily available to staff. Ensures Stroke Team staff members
communicate with patient or significant other regarding needs for
care. Quality and Performance Improvement: Coordinators develops
framework for maintaining and ongoing assessment of TJC measures
providing feedback in a timely manner. Maintains and enters TJC-AHA
required data and submits by deadline. Meets with Medical director
and quality regarding PI and QA projects. Community Outreach:
Provides EMS feedback to Director and assists with develop plans to
educate community on Stroke signs and symptoms. Participates and
leads Stroke education for our Community. Responsible for meeting
and/or facilitating designation for Primary and Comprehensive
Stroke programs as determined by TJC and state requirements.
Participates in AHA Get With The Guidelines data collection and
stroke core measures. Reviews and analyzes data and adjusts
programs as necessary. Demonstrates behaviors of service excellence
and CARE values (Compassion, Accountability, Respect and
Expertise). Performs all other related job duties as assigned.
Experience
Generally requires 5 to 7 years of related experience.
Education
Bachelor's degree in Nursing is required.
Skill
Ability to analyze, organize and prioritize work accurately while
meeting multiple deadlines. Ability to communicate effectively in
both oral and written form. Ability to handle difficult and
stressful situations with critical thinking and professional
composure. Ability to understand and follow instructions. Ability
to exercise sound and independent judgment. Knowledge and skill in
use of job appropriate technology and software applications.
Credentials
Licensed as a professional Registered Nurse in the State of Florida
or eligibility. American Heart Association Basic Life Support (BLS)
and any additional applicable life support certification for
Healthcare Providers is required upon hire with at least 6 months
validity and maintenance at JHS for the duration of employment.

Working Conditions
Physical Requirements - Job function requires extended standing or
walking. Must be able to carry objects weighing 21 to 50 pounds.
Able to lift or move objects weighing 51 to 100 pounds with the use
of appropriate devices. Jobs in this group require frequent visual
acuity to perform activities related to extended use of computers.

Environmental Conditions - Jobs in this group are required to

also be exposed to needle sticks, airborne infections, medical
gases, X-Ray, chemical exposures and other potential hazards. Must
wear Personal Protective Equipment (PPE) when exposed to
