Job Description:
A company is looking for a Case Manager LPN.
Key Responsibilities
Contribute to the assessment of health status and report changes in
condition to the supervising RN
Complete timely post-discharge patient outreaches and assist in
interventions for high-risk patients
Support and manage post-discharge needs to reduce hospital
readmissions and improve patient care engagement
Qualifications & Requirements
High School Diploma or GED required
Active California LPN/LVN License
EMR Software experience
