Job Description:
A company is looking for a VP of Value-Based Care
Operations.
Key Responsibilities
Lead the implementation of the CMS GUIDE Model and ensure
compliance with quality benchmarks
Develop strategic relationships with ACOs to enhance post-acute
care services
Oversee training and performance management for Care Navigators and
monitor program performance
Required Qualifications
Bachelor's or Master's degree in Nursing, Social Work, Public
Health, Healthcare Administration, or a related field
5+ years of clinical leadership experience, preferably in dementia
care or post-acute care management
Demonstrated success in developing ACO partnerships and value-based
care strategies
Deep understanding of CMS models and value-based reimbursement
frameworks
Experience leading training programs for care teams, including Care
Navigators and case managers
